Affordable Concierge Medicine: Direct Primary Care

We have eliminated the red tape and waste of insurance-based care and created our new membership medicine practice serving no more than 400 patient-members. We can meet your needs when they arise in the way that serves you best.
The monthly fee to become a member of Continuum Care is priced affordably and dependent on age:
- Ages 18-25: $75/month
- Ages 26-34: $125/month
- Ages 35-64: $150/month
- Ages 65+: $200/month
- Suboxone Treatment: $250/month
- Homebound: $300/month
Payments may be made monthly for your convenience, and discounts are available:
- Married Couples: $225/month for partners age 26-34
$275/month for partners age 35-64
$350/month for partners age 65+ - Family Plan: $200/month for one parent and one dependent age 12-25
$400/month for two parents and up to two dependents age 12-25 living at the same address
Children of members, birth to age 11, will be seen for sick visits free of charge as a benefit of membership
Although your membership medicine fee will cover the vast majority of your primary care needs, it is essential that you and your family maintain a medical insurance policy to cover those medical fees not included in your membership (such as hospitalizations, services provided by specialists, diagnostic testing and prescription drugs). Memberships are open to Medicare patients as well. You’ll love the attentive care and benefits:

- Extended office visits of 30 minutes or longer
- Home visits (“house calls”) if necessary

- $75 facials at The Spa at Mecca

As a patient-member, you get all of these benefits for the modest monthly membership fee. We offer new-member discounts and plans to suit most budgets. You get personalized, relationship-driven health care where Dr. Lacara provides the type of care once provided by the old-time family doctor for patients of all ages in Fairfield, Lincoln Park, Little Falls, Montclair, Cedar Grove, Essex Falls, Caldwell, North Caldwell, West Caldwell, Montville, Roseland, Wayne and Verona, NJ.
Our office policy is a 6-month minimum membership commitment; 30 day written notice required for cancellation of membership.
If your automatic credit card charge fails, there will be a $25 processing fee each time there is a failed payment. Two consecutive failed charges will result in a discharge from the practice.
