Your concierge medicine-style care at Continuum means you can talk to Dr. Lacara when you need to, on your schedule. Dr. Lacara makes herself available to her direct primary care patients 24/7 via phone, email, text or Skype. You can have same-day or next-day visits when you need them.

In addition to treating common and complex medical issues, Dr. Lacara takes the time to get to know you and your family as well as the interests, activities, stressors and goals that influence your daily life. If you have become accustomed to assembly-line medicine like so many patients today, you’ll appreciate Dr. Lacara’s relaxed, unhurried, extended office visits and comprehensive, personalized primary care.

Dr. Lacara believes in relationship-driven preventive health care where the practice size is kept deliberately small, enabling more attentive, convenient care just like the "old-time" family doctor provided. The doctor-patient relationship is the core of her practice. She truly partners with her patients as a family doctor, advisor, confidante and friend, always there for you when you need her.

Truly Personalized Care

Dr. Lacara feels she can be the best doctor when she has time to truly get to know you. That’s why her appointments are a minimum of 30 minutes while the typical assembly-line doctor spends only 7 to 10 minutes with you. Also, by limiting her practice to a few hundred patients, she can tailor your treatment and care specifically for you as an individual. Consider that the average primary care physician manages 2,500 to 3,000 patients, making it very difficult to provide high-quality personalized care.

Affordability

For the cost of a Starbucks coffee each day, Dr. Lacara will provide virtually all your direct primary care health needs. You won’t have to meet insurance deductibles or make copayments or worry about coinsurance. If you have health insurance, we will continue to bill them for covered expenses. In addition to the core services we provide, you will enjoy 24/7 access to Dr. Lacara, extended office visits of 30 minutes or more, deeply discounted lab and radiology pricing, plus comprehensive compassionate care, end-of-life care and house calls when necessary.
